A focused 60 minute virtual session with an RN credentialed care manager. You leave with a written Clarity Action Plan within 24 hours, your three highest priority action items, ranked. The full fee credits toward a Comprehensive Care Assessment within 14 days.
The Foundation of Everything
Comprehensive Care Assessment
$750 one time
A 90 to 120 minute care needs evaluation across eight domains: medical history, cognitive, functional, environmental safety, financial, legal, psychosocial, and nutritional. You receive the PureView Care Blueprint: a 15 to 25 page report with a 12 month care strategy, presented in a 45 minute family call. This is the required first step for every ongoing care management partnership.

Additional family members can be added to any care plan for $497 per month. Additional advocacy hours beyond your plan are billed at $250 per hour. Cancel any time with 30 days notice after your minimum term.
