Who we are
PureView Private Care Management ("PureView," "we," "us") provides private care management, care coordination, and healthcare advocacy services to families in Massachusetts. This Privacy Policy explains what information we collect through our website, how we use it, and the choices you have. It applies to this website and to information you submit through it. It does not replace any separate service agreement you sign with us as a client.
Information we collect
Information you give us directly. When you fill out our contact form, request our care assessment, download a guide, or join our email list, we collect the information you choose to provide: typically your name, email address, phone number, and anything you write about your family's situation. Our assessment asks general questions about the care situation you are navigating so we can respond with a relevant recommendation.
Information collected automatically. Like most websites, we use basic analytics that collect standard technical information: pages visited, approximate location derived from IP address, browser type, and how you arrived at our site. We use this to understand how the site is used and to improve it. We do not use this information to identify you personally.
Client records and health information. If you become a PureView client, you may share medical records, care documents, insurance information, and other sensitive health information with us so we can review, coordinate, and advocate on your behalf. We treat all client health information as confidential. It is collected, stored, and used only for the purposes of delivering the services described in your written service agreement, including future care coordination for the same client, and it is protected with encryption and access controls consistent with the standards established under the Health Insurance Portability and Accountability Act (HIPAA) and Massachusetts data security regulations (201 CMR 17.00), regardless of whether a given record is technically subject to those laws.
Use our secure channels for sensitive documents. Our public web forms (contact, assessment, downloads) are intended for general inquiry information only. Please do not submit detailed medical records, insurance numbers, or Social Security numbers through public forms. When you become a client, we will provide a designated secure method for transmitting records, and where we offer a client portal, records should be exchanged through it rather than by unsecured email.
How we use your information
We use the information you provide to:
- Respond to your inquiry, assessment, or discovery call request;
- Send you the guide or resource you asked for;
- Send occasional emails about new family guides and our services, if you joined our list;
- Deliver, document, and improve care management services if you are a client, including reviewing records you provide and maintaining them for ongoing and future coordination of the same client's care;
- Operate, secure, and improve our website;
- Comply with legal obligations.
Every marketing email we send includes an unsubscribe link. Unsubscribing stops marketing email; we may still contact you directly about services you have specifically requested.
What we never do
We do not sell your personal information. We do not rent or trade our email list. We do not share the details of your family's care situation with anyone outside PureView except as needed to deliver services you have engaged us for (for example, communicating with a provider at your direction and with your authorization), or as required by law. We never use client health information for marketing.
Service providers
We use a small number of vendors to operate this website: website hosting, email delivery, and website analytics. These providers process information on our behalf under their own security and privacy commitments, and only to provide their service to us. We do not authorize them to use your information for their own purposes.
Data retention and security
We keep inquiry and email list information for as long as it is useful to serve you, and we delete or anonymize it when it is no longer needed. Client records are retained for the duration of the engagement and afterward for as long as reasonably necessary for continuity of care, legal compliance, defense of legal claims, and professional recordkeeping standards, after which they are securely destroyed. You may request earlier deletion of your records, and we will honor the request except where retention is required by law or is reasonably necessary for the purposes above.
We use administrative, physical, and technical safeguards appropriate to the sensitivity of the information, including encryption of sensitive records in transit and at rest, access limited to personnel who need it to serve you, and vendor agreements with appropriate confidentiality and security commitments, consistent with Massachusetts data security standards (201 CMR 17.00) and HIPAA aligned practices for health information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. If a breach of security affecting your personal information occurs, we will notify you and regulators as required by Massachusetts law (M.G.L. c. 93H).
Your choices and rights
You can unsubscribe from our emails at any time using the link in any message. You may also contact us to ask what information we hold about you, to correct it, or to request deletion, and we will honor reasonable requests unless we are required to keep the information by law. Massachusetts residents may have additional rights under applicable state law.
Children
Our website and services are directed to adults arranging care for family members. We do not knowingly collect personal information from children under 13. If you believe a child has provided us information, contact us and we will delete it.
Changes to this policy
If we make material changes to this policy, we will update the effective date at the top of this page and, where appropriate, notify email subscribers. Continued use of the site after changes take effect means the updated policy applies.
